The West Baltimore RICH Collaborative team is seeking a Community Outreach Worker I (Contractual).
The Community Outreach Worker II (COW II)is responsible for helping neighbors navigate and access services.
The COW II will refer the neighbors to other members of the West Baltimore RICH Collaborative team for follow up with social and economic issues and increase awareness and need for health prevention related to hypertension and social isolation.
This position also participates in outreach and health screening activities organized by the West Baltimore RICH Collaborative.
Contacts and interactions vary and may involve multiple constituencies such as direct interaction with the West Baltimore RICH executive management, community organizers, the general public, patients, physicians, colleagues, assigned staff, vendors, contractors, and consultants for the purpose of providing and exchanging information.

All employees are expected to work primarily physically within the State of Maryland.
Primary Duties:
Provides crisis intervention through individual counseling, providing basic health education, assisting clients access various social services, and referring clients to alternate care facilities.
Conducts initial assessment in order to determine individual client needs.
Speaks before neighborhood groups and attends community meetings to establish communication and rapport; to promote services; and assist in resolving problems facing the community.
Prepares and distributes informational literature.
Participates in the evaluation of materials and methods designed to meet the educational needs of the client community.
May provide guidance and training over other outreach workers.
Establishes a supportive relationship with neighbors and their families to encourage their active engagement in their health care and support programs.
Assists with scheduling appointments as needed and assists neighbors to find appropriate resources.
Collaborates with care team or partners to help neighbors understand care plans and self-care management goals while trying to reduce cultural and socio-economic barriers.
Provides timely communication to the team and reports neighbor needs/issues to ensure coordination of care and appropriate referrals to other members of the care team.
Assists neighbors in completing applications or other requirements for accessing services and follows up to provide assistance to support neighbors accessing services.
Participates in focused outreach, community health screenings, and community events to promotes services and programs within the West Baltimore RICH Collaborative.
Engages in community-based events which occasionally occur in the evening hours and on weekends.
Provides referrals for community services as appropriate and in consultation with the care team.
Provides accurate and timely documentation and reports in compliance with UMB, partner sites and funder needs.
Performs all of the duties of a Community Outreach Worker I.

Minimum Qualifications Education:High-school diploma or GED.
Experience: Two (2) years of experience in community outreach, health care, research, and/or social services.
OTHER:Except for qualifications established by law, additional related experience and formal education in which one has gained the knowledge, skills, and abilities required for full performance of the work of the job class may be substituted for the education or experience requirement on a year-for-year basis with 30 college credits being equivalent to one year of experience.

Ability to communicate effectively orally and in writing.

Ability to maintain poise and courtesy under pressure.
CONDITIONS OF EMPLOYMENT: Employees in this job class may be required to use their personal automobile while conducting official business and possess a valid Maryland Non-commercial Class C or Commercial Class B Driver's License (CDL).

Employees in this job class may be required to successfully pass a police background check.

HIRING RANGE:$20.51/hour - $23.00/hour, commensurate with education and experience.
